Make the form work on paper
Keep the form on one page with readable type and enough space for comments. Put the rating labels beside the questions rather than only on a separate instruction sheet. Provide a private return point such as a collection box. Guests should be able to give feedback without having to explain a low score in front of staff. If you need a response, invite them to leave contact details voluntarily.
Turn it into a digital or QR-code form
Use one question per field and keep the same labels as the paper version. Choose radio buttons for ratings and separate text fields for comments. Show the contact field only when a guest wants a reply, if your tool supports that. Test the form on a phone before printing the QR code. Check that guests can answer without creating an account. A QR code is only a link to the form; it does not make the form private or anonymous.
Offer a brief hotel satisfaction questionnaire when needed
The five ratings on this card are a starting point for overall satisfaction and operational follow-up. If you need to evaluate Wi-Fi, food and beverage, facilities or arrival separately, use the detailed survey-question bank and include Not applicable for unused services. Keep that longer questionnaire separate from this short comment card so the purpose stays clear.
Collect feedback without selecting only happy guests
Offer the form consistently to eligible guests. Do not make it available only after a positive conversation or a high rating. Keep feedback voluntary. For an in-stay problem, use a staffed contact channel so urgent requests are seen in time. This form should not promise an immediate response unless your team can provide one.
Read comments and close the loop
Assign a person to review submissions on the schedule stated on the form. A serious hygiene, safety or access concern may require action after one report. For routine service issues, record the theme, investigate it and track whether it returns. Reply when requested, then note what changed. Be clear about contact details
Guests do not need to identify themselves to leave useful feedback. Tell them who will read the feedback and why you ask for contact information. Use contact details for the requested reply and do not treat them as a marketing signup.
